Replit, Lovable, Bolt, and v0 wire the form themselves from that prompt. Already have a contact form in your build? The prompt tells them to connect the existing one instead of making a new one.

These builders can't connect MCP servers, so the prompt is the integration. It works because their agents write your frontend anyway.

On WordPress: paste it into a Custom HTML block

In the page editor, add a block, search "Custom HTML", paste the snippet, update the page. Using Elementor or Divi? Same thing with their HTML widget. No plugin needed, nothing to install.

Any input type works: text, radios, selects, checkboxes. Every field gets emailed to you and stored in your inbox. Submit the form once yourself to see the whole thing land.

Field	Effect
subject	Sets the notification email subject.
redirect	URL to send the visitor to after submitting. Without it they get a clean thank-you page.
email	If present, replying to the notification goes straight to the sender.
botcheck	Honeypot. Render it hidden and empty; bots that fill it get silently dropped.
